Program Guidelines
– Patient portion of the bill is due at time of scheduling
– Program cannot be used in conjunction with
other dental plans or programs
– No refund of premiums will be issued if participant decides not to utilize dental plan
– Program is only valid at 3D Dental.
– Program cannot be used for referral to specialist or for hospital care
– Program cannot be used for treatment for which, in the sole opinion of our doctors, lies outside the realm of their capability
